閣下提供的資料，為富衛保險有限公司("FWD")提供保險業務所需，並可能使用於下列目的 ． 任何保險或財務有關的產品服務，或該等產品或服務的任何更改、變更、取消或續期； ． 任何索償、或該等索償的調查或分析；及 ． 行使任何代位權；及 可能移轉予： ． 任何有關的公司，或任何其他從事與保險或再保險業務有關的公司，或與保險業務有關 中介人或索償或調查或其他服務提供者，以達到任何上述或有關目的； ． 現存或不時成立的任何保險公司的協會或聯會或類同組織(「聯會」)，以達到任何上述 或有關目的，或以便聯會執行其監管職能，或其他基於保險業或任何聯會會員的利益而 不時在合理要求下賦予聯會的職能；及 ． 或透過聯會移轉予任何聯會會員，以達到任何上述或有關目的。 此外，在此授權FWD由聯會從保險業內收集的資料中查閱及/或核對閣下任何資料。 閣下有權查閱及要求更正由FWD持有有關閣下的個人資料，如有此項要求，可向FWD之資料 保護主任提出，地址為香港德輔道中308號富衛金融中心1樓。 The information you provide to FWD General Insurance Company Limited ("FWD") is collected to enable FWD to carry on insurance business and may be used for the purpose of ． any insurance or financial related product or service or any alterations, variations, cancellation or renewal of such product or service; ． any claim or investigation or analysis of such claim; and ． exercising any right of subrogation; and may be transferred to: ． any association, federation or similar organization of insurance companies ("Federation") that exists or is formed from time to time for any of the above or related purposes or to enable the Federation to carry out its regulatory functions or such other functions that may be assigned to the Federation from time to time and are reasonably required in the interest of the insurance industry or any member(s) of the Federation; and ． any related company or any other company carrying on insurance or reinsurance related business or an intermediary or a claim or investigation; or other service provider providing services relevant to insurance business for any of the above or related purposes ; ． any members of the Federation by the Federation for any of the above or related purposes. Moreover, FWD is hereby authorized to obtain access to and/or to verify any of your data with the information collected by the Federationfrom the insurance industry. You have the right to obtain access to and to request correction of any personal information concerning yourself held by FWD. 	就有關從投保書所收集的個人資料： 1. 在申請及接受保險產品、服務及相關的其他服務時，閣下有需要不時向「教安心」 提供個人資料。 2. 「教安心」亦可能會在日常業務運作中向閣下收集個人資料，例如當閣下提出更改 保單內容及申請保險索償等等，以提供、維持、管理和操作該產品及服務。 3. 閣下的個人資料可能會用作下列用途： 3.1 處理及評估保險產品及服務的申請； 3.2 向閣下提供後續服務，包括但不限於執行及管理已發出的保單； 3.3 處理閣下就「教安心」的保險產品及服務提出的要求，包括但不限於要求增加、更改 或取消保單、保障項目或受保人，及安排繳付保費等等； 3.4 處理索償及相關的行動； 3.5 執行與提供的保險產品及服務相關的行動，例如核實身份及資料配對等等； 3.6 行使「教安心」向閣下提供保險產品及服務而享有的權利，例如向閣下追討欠款； 3.7 為遵守任何法例的要求，或根據監管或其他機構所發出對「教安心」具有約束力或 要求其遵守的規則、規例、手則及指引而作出披露。 3.8 製作统計數據及進行市場研究； 4. 閣下的個人資料可能會用作下列產品及服務的直接促銷及優惠推廣： 4.1 保險產品及服務； 4.2 獎賞、會員或優惠計劃和相關的產品及服務； 4.3 與上述有關的其他用途。 5. 上述產品及服務會由下列機構提供及/或直接促銷及優惠推廣： 5.1 「教安心」(包括：聯誠保險代理有限公司、聯誠保險管理顧問有限公司) ； 5.2 保險產品及服務的承保保險公司； 5.3 向「教安心」提供支援的第三方獎賞、會員或優惠計劃提供者； 5.4 第三方推銷服務提供者。 6. 就有關從投保書所收集的個人資料，除非於投保書上另有訂明，投保書上所提供的 個人資料將會以下列方式： 6.1 電郵地址； 6.2 聯絡電話以短訊形式； 6.3 傳真號碼；及 6.4 通訊地址 向投保人及受保人傳送「教安心」提供之保險產品、服務、優惠及有關的資訊。 7. 根據條例規定，閣下有權： 7.1 查詢「教安心」是否持有閣下的資料及查詢有關資料； 7.2 要求修改投保人及受保人的個人資料； 7.3 要求取消以投保人及受保人的資料作直接促銷的用途。 投保人及受保人可就上述第7項，隨時以書面或電郵方式聯絡「教安心」的個人私隱主任
